The earliest occurrence seems to be Episode 112 of Etho's primary let's play series. In Landmark (Etho Plays Minecraft, Episode 197), Etho stated how he didn't want the quote to become the normal intro, and that people got upset every time he didn't say it. In 1 Million!! (Etho Plays Minecraft, Episode 283), Etho mentions how the quote became unintended, how he doesn't know what people like.
